Implement per-monitor settings

This is a complicated one. NotchNook currently only supports hiding NN on non-notched screens. We need to have a per-monitor toggle that uses some unique identifier for each monitor and remembers the settings even if the monitor is disconnected.

The settings for this feature should be cute.
